Information about you
Complete this short survey to see if you meet the criteria of Sarge's House mission. Most applications will be reviewed and answered within 48 hours. If you require a long weekend stay for your pet or several weeks or months, we will put your application in the highest consideration to meet your needs and allow your pet the best time ever at Sarge's House.
Prior to completing the application for your pet, please be ready to share veterinary information (including up to date vaccine records), animal behavior (all behaviors will be considered by our organization, please be honest).

What to expect after you apply
After we receive information about you, you will receive a phone call from us within 48 hours to talk about next steps. If approved, you will receive a form for each pet to determine which location is best for you and your pet.
